Immune System Instead of Disease behind Marginal Bone Loss around Dental Implants

Several Swedish well published authors evaluated marginal bone loss caused by titanium alloys (this includes “commercially pure” titanium) compared to the commonly held belief that bacterial actions (micro leakage at the abutment/implant interface) are the primary cause of this crestal bone loss. Albrektsson et al, Int J Oral Maxillofac Implants, 2020. Their conclusion was that the osteoclastic crestal bone loss is primarily caused by the chronic inflammation, likely caused by the various metals causing corrosion and secondarily by bacteria.
Treating Peri-implantitis By Dr Kusek Once peri-implantits has invaded the bone (a 6mm PPD), and is non-responsive to conservative measures (carbo frequency minimization, oral hygiene and professional hygiene), it is expected that surgery is required to stop the disease process. Innovative treatments such as closed flap laser treatment (LNAP & Biolase) show varied results at this stage. During surgery the biofilm on the rough implant surface can be removed with 1) glycerin & air polishing, 2) or a Erbium or 3) a Co2 Laser. The worse lasers for implant decontamination are diode and Nd:YAG as they create excess heat in the metal implant. Multiple surgical steps are additionally needed to remove granulation tissue, decorticate the surrounding bone, chemically treat the biofilm toxicity, regenerate bone, regenerate soft tissue and stimulate healing.
