Chlorhexidine use with Peri-implantitis

Chlorhexidine is an effective anti-microbial agent. A recent publication by Zhao et al, in the J of Oral Implantology in 2021 involved a meta-analysis of 6 randomly controlled trials of 207 peri-implantits patients. No significant effect was found with BOP, PPD reduction or CAL gain when mechanical debridement was used with or without CHX.
Since no notations were made about swishing with CHX, or brushing with it or irrigating with it, I’ll assume that was a variable not accounted for. When working with periodontal disease around implants, and teeth we have switched to Vega by Stella Life. It is a combination of essential oils and it has no effect on fibroblasts. A recent paper (which I don’t have at hand) showed it out performed CHX as well as 2 other most commonly used mouth rinses.
We encourage peri-implantitis patients to use it at the end of the brushing/flossing/ water irrigation, and not swish with water after using it. I recommend waiting 2 months after any surgery before having patients return to using their irrigation devices.
